She is being shady about it. If it truly was a medical issue that is preventing her from traveling she would've provided the necessary documentation when she first tried to cancel the reservation. The cancellation policy is spelled out and if she had coughed up the proper paperwork without being asked for it it would be a different story. Typically you will get some type of note with your discharge papers that indicates your restrictions for work and travel. I've been given one every time I injure myself and have to see a doctor.

This exactly. Even the representative with the company agreed that it was shady and questionable, based on the interactions, but that they wouldn't fight it. I could have tried to fight it but it's not worth the headache.
I've never used AirBNB, but don't you have the option to give users a rating, like if they were a horrible user, so others would know not to rent to her? I'd take advantage of that if possible.
And, maybe the AirBNB rep you were talking to, who agreed that the renter was being shady, would refund at least part of their fees?
