How to stop overprescribing antibiotics

ANTIBIOTICS are an indispensable weapon in every physician’s arsenal, but when prescribed unnecessarily for nonbacterial infections like the common cold, as they too often are, they provide no benefit and create problems. They wipe out healthy bacteria and can cause side effects like yeast infections and allergic reactions. Worse still, they contribute to the rise of “superbugs” that resist antibiotic treatment.



The Centers for Disease Control and Prevention estimates that about half of outpatient antibiotic prescriptions in the United States are unnecessary. It also estimates that each year as many as two million Americans suffer from antibiotic-resistant illnesses, and 23,000 die as a result.



Clearly, we need to get doctors to prescribe antibiotics more selectively. But how can this be done?



Several strategies have been tried in recent years, without much success. Educating doctors and patients about the proper use of antibiotics has had only a modest effect, as most doctors already know when antibiotics are called for. Alerts sent to physicians through the electronic health record reminding them to not prescribe unnecessarily are often ignored because doctors are overloaded with such messages. And offering doctors financial incentives has had mixed results, in part because the payments are modest relative to a doctor’s salary.



These strategies are all based on the assumption that physicians are rational agents who will do the right thing if provided proper information and incentives. But what if doctors are a little irrational, like the rest of us? They may overprescribe antibiotics out of an unrealistic fear that the patient could eventually develop complications and need them, or because it is easier than arguing with a patient who insists on getting them. (Doctors appear to take the path of least resistance as they get more tired. In a study published in JAMA Internal Medicine, we found that prescription of antibiotics increased over the course of four-hour clinical shifts, whether or not it was called for.)



Over the last few years, our research team has developed several new approaches to reducing unnecessary antibiotic prescribing, drawing on insights from behavioral economics and social psychology. These disciplines acknowledge that people do not always behave rationally and are strongly motivated by social incentives to seek approval from others and compare favorably to their peers.



In one study published a few years ago, we asked a group of doctors to place a signed poster in their exam rooms pledging to follow standard guidelines on antibiotic prescription. This tactic, which pressured doctors to act consistently with their own publicly stated commitments, reduced inappropriate prescribing 20 percentage points relative to doctors in a control group who displayed a poster with generic information about antibiotic use.



In a more recent study, we showed that doctors tended to prescribe less aggressive medications when such options were presented more prominently (one by one, in a vertical column), with more aggressive options presented less prominently (grouped side by side, in a single category). Previous research suggested that listing alternatives individually made them appear more popular — and therefore more appropriate — than when they were grouped together. And indeed, we found that doctors were roughly 12 percent less likely to order more aggressive medications, such as antibiotics, if these options were grouped together, compared with when they were listed individually.



For our latest study, published last month in JAMA, our team gathered data on prescription rates of 248 clinicians at 47 primary care practices in Los Angeles and Boston over 18 months. Then, over another 18-month period, during which we focused on nearly 17,000 acute respiratory infection cases in which antibiotics were not called for, we tested new approaches to promoting more selective prescribing.



In one approach, doctors received a monthly email informing them of their performance relative to that of their peers. Those with the lowest inappropriate antibiotic prescribing rates were congratulated for being “top performers.” Doctors who were not top performers were told “You are not a top performer.” The email also included a personalized count of unnecessary antibiotic prescriptions and the count for a typical top performer. This “peer comparison” approach almost completely eliminated inappropriate prescribing: from 19.9 percent in the pre-intervention period to 3.7 percent during the post-intervention period — an 81 percent reduction.



In another approach, whenever doctors prescribed an antibiotic that was not clearly called for by the diagnosis, the electronic health record system asked them to provide a short “antibiotic justification note.” The note would be entered into the patient’s medical record and would be visible to others. Introducing this speed bump into the work flow, along with the prospect of social accountability, reduced the inappropriate prescribing rate from 23.2 percent to 5.2 percent — a 77 percent reduction. (Our control practices also experienced reductions, possibly because doctors knew they were being monitored, but our two approaches were much more effective.)



Taken together, our studies suggest that simple and inexpensive tactics, grounded in scientific insights about human behavior, can be extremely effective in addressing public health problems.

It would be great if medical professionals (and their patients) could be satisfied with fewer prescriptions for antibiotics, but it's a drop in the bucket compared to the overuse and abuse of abx by the agriculture industry.

Agreed - I don't think we'll see any major changes in resistance problems until we address ag use.

We also need to do a better job at patient education. Just because your snot is green, doesn't mean you need a z-pack. In the age of patient satisfaction surveys and online reviews, judicious physicians can get dinged for appropriately not prescribing abx.

They are definitely against handing out antibiotics in England. I think I've gotten them once in the 11 years (and I'm ill almost every winter).  My GPs have said as much that they only want to hand them out if it is utterly necessary.

My pharmacy is right beside a walk in clinic. So antibiotic overload, right? Nope. The doctors are really great at rarely giving antibiotics. They do a lot of nasal sprays and inhalers to help symptoms but I always tel the patients that do get antibiotics that they must be super sick.

There is also some information that not every UTI needs and antibiotic. But until they can determine which one is going to develop into a kidney infection, I have to agree that over prescribing is okay.

Seriously, though, we are not seeing the forest for the trees, here. Aren't over 80% of all antibiotics routinely fed to livestock? This is much like being told to water their rose bushes less during a drought when residential water consumption is only 10-20% of all water consumption. Yes, it's important to do what we can, but there's a huge piece of the puzzle not being addressed, here.

I've seen a lot of changes in the time I entered nursing till not. UTI's aren't always treated. Only when symptomatic. Ear infection symptoms need to be present for 2-3 days before a prescription as well.
