We went when DS was 16 months. Overall he did great and enjoyed more than I thought he would. Sometimes he napped in the stroller, sometimes we went back to the room.

We drive from NE Ohio. (14-15 hours on day 1 and 2.5 on day 2), We've done it at ages (12 months x2), 18 months and just shy of 3. It was absolutely fine.
We definitely needed room naps at 18 months.
The trip at 18 months had its challenges but we still had a blast. My kid had an opinion at that age and making sure we 'listened' to her helped when it came down to what she wanted to do. We people watched A LOT that trip. It's fun to see how they react to everything at that age.
We go back for pool & rest time regardless of the kid's ages. DH & I did it before kids most days. I think the trickiest part will be touring w/ the other family since they won't have a toddler, too. I think you might be able to get away with a nap on the go a day or 2 but I would have your whole family go back to relax & swim a few days, too. One parent can hang out in the room & you can swap. I think you have to be careful that the toddler isn't doing a a lot of waiting around while the bigger kids ride the bigger ride - luckily at Disney the toddler can still ride most things. It will be very different than last time but adjusting your expectations is half the battle.
I don't think the drive will be as bad as you think. The older kids can help entertain the toddler.
We are in a similar situation. My kids will be 5, 3, and a few days short of 1. We are going for a full 7 days, so I'm hoping we can just take it slow, since we are also skipping HS. I plan on taking breaks everyday to use the pool/nap/rest. My kids are early risers, which means we will hit the parks early and be done by the afternoon. I made early dinner reservations (btw 4:30 and 5), too. I'm planning on renting a double stroller. Between the stroller and my baby carrier, I hope the 1 year old is happy. i'm not into the thrill rides, so DH will take the boys, if they want to go, and I'll keep baby entertained. I just know we aren't going to get to everything and I'm fine with that.

Is your 3 year old tall? at a couple days shy of 3 there was only 1 ride (barnstormer. Just missed the height for mine train) that my oldest could do that my almost 1 year old could not.
I just did an 18 hour car trip with a 6 year old and a 22 month old (each way). my mom flew and met us there. I'll be honest, it was tough to have the 22 year old (more in the parks than in the car). My mom spent a lot of time back at the hotel with him. If you could stay at a monorail resort or take a grandparent that would be my suggestion.
I wouldn't take a 19 month old if it meant a 14 hour car ride.
We would break it up into a 2 day trip (each way) Any better?

I am super breezy about tv so my 18 hour road trip with an 18 month old was a mix of movies, naps and snacks. Drive down was so easy. We did have a hell period on the way home of 2 hours of screaming from Atlanta to TN but that's our fault because we visited friends until super late and should have just stayed in Atlanta.
At 13 months DS did AWESOME at Disney. Disney was one of the only places I could ever get him to nap in his stroller. The only issue we had was he was incapable of staying up for night time shows and we weren't on the monorail so we didn't get to see any fireworks that trip.
We're going to DLR next month. DS will be a few days shy of 2. I think you'll be fine. We bought plane tickets to avoid a road trip though because just the thought of a 13-14 hour drive with a toddler made my husband want to run screaming into the night. Is flying not an option?
