Yeah, I usually don't think about it either. I'm trying to imagine doing it right now to think where do my hands naturally go to? I guess I usually lift from near my temples/in front of my ears. But in my heavy swimming days, I have always ended up with some wispies from caps being hard on my hair.
It helps if you dunk your head under water just as you're sliding it off! Pull the sides away slightly just as you're going under, and water will bubble under the cap and help pull it off. Hard to explain... but watch some videos of swimmers after races (i.e. The olympics).
I slip my fingers underneath in the middle of my forehead, and pull it back. It turns inside out as I pull it off, same as how you remove disposable gloves.

this is what I do, didn't even think of taking it off while actually in the pool! I will do that tomorrow

Nope. Ponytail holders under my cap give me headaches. So I pull all my hair to the back and twist it so it sort of stays together, put on my cap, and then stuff all the hair under. It's not a very glamorous process.
