For me, lanolin plus airing out worked best. As in, after feeding, walk around topless as long as possible, all the way until the next feeding if possible. The smoothies did feel great, but they weren't anything magical.
Both of mine clusterfed like mad in the evenings, and it drove me up a wall. But it did pass, and I just spent a lot of time on the nest for awhile, lol.
The best thing I found was expressing milk onto my nips and walking around shirtless or (if I was worried about company) with the panels of my nursing tank hanging open
ETA: earth mama angel baby nipple butter is edible so you don't have to wash it off pre-feed and it's quite good too

Mine were so cracked and damaged that I was crying when he would latch, so my LC suggested lanolin mixed with a teeny bit of polysporin to heal the damage. It does need to be wiped off before they nurse again (polysporin is not okay for them to ingest), but it did wonders in quickly healing the nips so that we could continue on with nursing. By that point they had toughened up enough that I didn't have any subsequent issues. GL!
Earth Mama Angel Baby Nipple Butter and letting them air out as much as possible. Like i'd slather on the butter and sleep with my boobs exposed to the air with the sheet pulled down. Hang in there, this is the roughest part. Don't quit, it gets so much easier!
I used lanolin with every feed and tried to let my nipples air out a bit. I used the gel pads when I was desperate (burning pain.) You'll toughen up pretty quickly- hang in there!
Random nipple-preparedness question: I have in my head somewhere that I should be "roughing up" (or was it "toughening up"?) my nipples in the 3rd trimester in advance of breast feeding by rubbing them with a wet washcloth in the shower during the 3rd trimester. Is this true?

I would not. Trust me, nothing is going to prepare you for having a tiny miniature person sucking on your nipples for hours on end. They will toughen up when the time comes. It will be fine, I promise! The pain of BFing is short-lived, it gets soooo easy after that short period has passed.
@idaho - I was told in our prenatal class that they don't recommend roughening them up anymore. Probably the best thing you could do is study up on what a good latch looks/feels like and get the hospital staff to help you as much as possible when latching on. Minimize the damage from the get-go
