I’m working with an online trainer right now but she charges way more than for the service I’m looking for. She does nutrition, check in, recipes, monitors my food on MFP, etc. plus created the workouts for me…but I don’t want all that. Just the workouts.
I want an app that knows what equipment I’m going to use (barbells, dumbbells, machines) creates the workout for me each day, can offer substitutions, keeps the data from each workout and prompts me for how much weight and how many reps to use, and then also has videos to demonstrate each task.

I'm not sure if it goes into that much detail for you, but I use FitOn. It's free (but has a paid option) and it's got a ton of great workouts and great instructors. I love it.

I used MacroswithEm. It's an app that has all the workouts and stores all the data for me. There's no thinking for me. It's all in the app. It's about $20 per month.

Stronger by the day?
The question mark is because I'm not sure if you're saying that each day you want to tell the app what equipment you have that day (like if you're at home vs the gym) and then have it suggest a workout based on that. If so, that's not going to be the app for you, but also that isn't the best way to get stronger. This app runs multi-week cycles programmed to increase your strength, and I know there are videos and substitutions.
