We had our puppy altered yesterday. She'll be keeping the cone on until it looks healed. She will not leave it alone. I agree, the cone hurts coming at you. The noise of it hitting things is driving me crazy, but I know it's probably deafening for her.
I think Quincy was in his cone for 7 - 10 days. He was a dedicated licker though. After the second or third day, we gave him a cone break each evening when we were able to be 100% with him to stop him if he tried to lick. Then it went back on for bed. Once he stopped trying to lick during his cone-free time, we let him go without it while we were home. He wore it when we went out and at night for a few more days after that just to be sure.

As regards the cone hurting/being loud - go to home depot. Buy a $1.50 length of self-adhesive pipe insulation. Apply around the outside edge of the cone (you may need to make a couple "relief" cuts, depending on how big the cone is). Voila!
I did this when Dot was in the cone for three weeks and if I hadn't, there would have been carnage - I was getting all bruised up.
