Every once in a while there's a box of tissues in the bathroom, but other than that, I've always had to byo. (This is the case at the 3 law firms I've worked/interned for, and the 3 large corporations I interned for). My allergies are such that I go through an astonishing amount of tissues, so I can't say I blame them.
We get free tampons though, which even as a Diva Cup user I think is thoughtful. And there are like 15 types of pens we can choose from.
I voted no, because I've only had one job that did. Not sure about the new place; I'll cross my fingers, but I won't hold my breath.
The job that provided tissues actuall provided so much stuff, I was still in awe after I'd been there a few months. Off the top of my head, I remember all this: tissues tampons pads coffee (single-serve machine w/probably 15 varieties) tea Crystal Light (three flavors) seltzer water (four flavors 10-15 varieties of canned soda (both Coke and Pepsi brands) hot chocolate (regular, diet and w/marshmallows) soup oatmeal fresh fruit cereal (three kinds) various pain relievers and health insurance (no premiums, no copays)

my office rpvides rough tissues - which I use. they also provide most of the stuff above depending on which building/floor you're on
I provide my own and I do not share. I would go through a box a day on the public desk in the youth room of my library. I keep them on my cart. When a mom walked behind he desk and grabbed some, I told her, "Those are not for public use, you'll need to go to the public restroom if you don't have any tissues with you." She called me a cheap bitch. Maybe, but keep your hands off my Puffs you whore (obviously, this was not my response).
I've never seen tissues provided at work. (I can't even remember if any of my jobs left a box in the bathroom.) I've always brought my own.
I've had jobs that provided free feminine products, coffee/tea/cocoa, milk and half-and-half, bottled water. All have provided basic office supplies. One place offered deeply discounted vending machine snacks and sodas (25 cents for a Coke). I used to work for a newspaper and our publishing deadline was Friday around midnight, so they'd pay for our dinner that day.

No it is against state policy to use $$ for tissue. Once before I knew that I ordered some for the office and my purchasing card was suspended for a month.
We either take up a collection or use non-state funds.
I usually bring my own because people who have to see me are often times emotional/in tears and its just one small comfort I can provide.
