Wherever. I always made sure they went to the front for my boy. You could probably buy larger inserts if you're not getting enough protection from the small ones. Or use the inserts from your larger size diapers. (I'm not really familiar with what comes with the sized FB as mine are all OS)
ETA: you can always double stuff them too. I've had to use 2 inserts almost from the get-go as ds is quite the wetter
All my inserts are mixed up but I don't worry about placement with any of them. I haven't noticed a difference. I think they'll kind of settle in the middle anyway.

I only have OS but have more small inserts than large. When I use the small inserts by themselves I either use 2 & overlap so they cover the whole diaper or I use 1 & put it in the middle.
I keep debating about buying a few more large inserts, they go all the way from front to back in the OS. Do you have the minky inserts? They are my favorite, no stains

We have both brands, I like them both for different reasons. Here are my reviews after 2mo of use.
Fuzzibunz OS Elites: -very trim (downside: probably won't fit as long as other OS dipes) -minky inserts are great (absorbent, trim & no staining) -good fit-nice that you can independently adjust leg & waist elastics -can be a little harder to stuff because pockets are narrow & PUL is stickier than other brands
Bum Genius 4.0s: -generous sizing, should fit for a long time -they are longer & wider than FBEs (downside: a little bulky) -good fit, snaps are easier than FBEs, I like that the tabs are also stretchy (instead of just the waistband) -microfiber inserts- absorbent & easy to wash but stain more easily. -easy to stuff
I have tried both NQB. For the newborn stuff I prefer FB - my baby is pretty small in the lower half and I think they fit her better.
so far I like bumgenius better for larger. The FB smalls were too big even though they said 7-18 pounds and she is 8.5. We had our first poop leak this morning - but maybe things will get better when she grows a little more.
. My 21 pounder still wear fb smalls and is nowhere close to outgrowing them. I can see them being too big.

Yeah, and for different occasions, too. I tend to grab FB for the diaper bag because they're smaller, or to put on under a cute outfit because they're trimmer. But I like BG for nighttime because I think they are more absorbent and less likely to leak.
I never really worried about the placement. However, DD outgrew the absorbancy of the FB smalls before she outgrew the diapers (she could probably still fit in them actually.) I ended up swapping out all my MF inserts for hemp.
I loooooved FB size smalls at first, but they started looking really worn out after a few months. I ended up switching over to BG 4.0s and prefer those now. FB smalls are awesome for the in-between phase when NB diapers are too small and OS diapers seem to big.
I may try Kawaiis and I'm interested in Blueberries too.
really I just need some more for daycare if we go with the in home. I use fitteds + covers or prefolds at home 90% of the time. Just using the 4.0s regularly right now to try to figure out if the rash was a reaction to them.

When DD was tiny, I always thought her skin seemed a little pink after being in BGs. I had a whole stash of BG 4.0s and sold all except two and switched over to FB. Tried BGs again a few months later and we had no issues! So, I bought a whole new stash of 4.0s and am now going to sell all my FB. Ugh.
Another mom told me that her LO outgrew a suede cloth sensitivity, too.
