We only have one left on each side and both are in their late 80s. My Grandmother is getting a nice pair of organic cotton PJs. MH's Grandfather is getting a Harry and David fruit and cheese assortment.
i only have my grandmom left. I had DS (2 years old) put his handprints on an oven mitt for her- it came out cute, and i feel like its something she might use. She also really likes wine, so i got her a bottle of wine, and a gift card for Cheesecake Factory (her and her "boyfriend" like to go there for dinner :-) )
My grandma's list includes things like toilet paper, paper towels, and gift certificates to Subway. We are getting her a toothbrush, toothpaste, and a grocery store GC.

nothing- my grandmother is 94yo and about to move out of her house (she has lived alone in all this time).... so she said she didn't want anyone to get her anything b/c it would just become another thing to pack, etc.
One grandmother just barely scrapes by financially and still cooks for a bunch of sons/grandsons who come to her house for lunch and dinner, so she's getting assorted bulk groceries from Costco (cornmeal, cinnamon, Graham crackers). I also brought her a box of lemons from my tree in CA. She's in GA, so lemons are expensive here. She tends to hoard, so giving her things that will save her grocery money and not add to her junk is the best thing we can figure to do for her. My sister got her sugar, flour and other spices.
Other grandmother is getting wool scarves because she loves them.
My grandmother is getting jammies, which she asked for. My grandfather is getting a couple books ( a Reader's Digest compilation, and one about WW2 that I thought looked really interesting). I'll also get them some snacks-- fancy nut mixes and that kind of stuff.
They usually like puzzles, too, to keep them busy in the winter, but I think my sister in law got them that.
They just built an addition with a second entrance on their cabin, so I got them a welcome sign to hang on the new door.
If they hadn't built that, then I would have probably bought a Christmas pin for my grandmother and some type of tool or geeky gadget for my grandfather. (He works in IT and is very geeky.)
I don't have any living grandparents left but DH does. Since they don't need anything each year when I order my Christmas wreath from the boy scouts I also order a centerpiece that is delivered to their house. The boy scouts send it UPS or FedEx and it always arrive around the 10th of December or so. His grandparents are able to enjoy it all season, are surprised with a package and we don't add clutter or crap to their tiny house (since again, they don't need anything).
My grandmother wanted a red or blue sweatshirt..so she is getting those and always a grocery store gift card. She cooks for our entire family almost every Sunday and with the holidays being so close together here at the end of the year..I know its difficult on her fixed income.
DH's grandparents are getting a restaurant gift card for the place close to their apartment.
Mine are gone, but my parents are old. We drew names, but typically dad gets booze (he likes some terrible plum brandy) and trail mix. I did get mom a timer for her light display as she was complaining about going out to unplug it.

DH's grandparents got a Yankee Candle and a Cracker Barrel gift card (we give them the same thing pretty much every year). My grandpa got a coffee mug with pics of all his great grandkids and a book about University of Nebraska.
