At my practice (I see midwives) the first appointment is with the nurse at 7 weeks, and then I went to my first appt at 9 weeks, which included a dating ultrasound. I just had my NT scan at 12.5 weeks.
With my current pregnancy, we didn't get an ultrasound until 10 weeks!! My first OB appt was at 8 weeks, but it was just an exam and reviewing their guidelines for pregnancy and the appt schedule. I didn't even hear the heartbeat at the 1st appointment, which was so disappointing.
My practice does 8 weeks for the first appointment. I went at 6.5 weeks with DD because I have long cycles, so they thought I was 8 weeks based on LMP. I went in right at 8 weeks with DS. It was actually worth the 1.5 week wait because we saw more baby (he even moved) than we did at the 6.5 week appointment with DD.

My practice has you come in no later than 8 weeks for a dating ultrasound and an appointment with one of the nurse practioners, where they give you an overview of the tests that are available (NT scan, etc. -- this appointment is short and largely a waste of time as the OB reviews everything with you again at your first appointment with him).
First appointment with OB is at 10 weeks with pelvic exam, legally required blood tests for STDs, etc.
Eight weeks is standard at my OB practice. Two of their offices do a dating u/s at the first appointment; the third office doesn't have an u/s machine so you'd have to wait for your NT scan for an u/s if you book your first appointment there.
I had an RE so after beta I did u/a at 6 weeks and then every 2 till nt scan. My OB usually does nt, anatomy and then one around 32 weeks. When I get pg again if I'm not with an RE I will request an early one for piece of mind.

For DD I had a dating ultrasound, NT Scan, Anatomy Scan. This time I have a better idea of my conception date so my practice doesn't do one until Anatomy Scan (!?!??!?!?!). We may do an elective NT.
My first appointment was at about 9 weeks (they said anywhere from 8-10 weeks). They just did labs and went over information. I had my first u/s at 12 weeks but only because I had spotting. Otherwise my only one would have been the a/s.
ETA: we could have done an NT scan but decided against it. I wasn't going to do any blood tests either since we are so low risk for the things they test for, but ended up doing the quad test. And we first heard the heartbeat at 12 weeks also, they didn't even try before then.
Mine were both at 8 weeks. First time I had a dating u/s after betas at 6 weeks because of a previous m/c. This time they did an ultrasound in the office with a new machine they had just purchased.

What does "NT scan" stand for? What is it?
NT stands for Nuchal Translucency. It's an ultrasound to check the fluid at the back of the babies neck as larger amounts of fluid could indicate chromosomal abnormalities. It can only be done at around 12 weeks because after that they are no longer to see the fluid levels there. They'll also check for a nasal bone as lack of that is another possible indication of an abnormality.
I went in around 4 weeks for a pee test and bloodwork. I just went back at 6 weeks 5 days for an ultrasound. I go back in 4 weeks and she said she will probably do another ultrasound because it is easier than finding a heartbeat on the doppler at 10 weeks. We aren't doing the NT scan, but that would be next. Then the anatomy scan.

1. 5 weeks at the RE, 10w at the OB. Everything at the RE was fine. OB couldn't find a HB with the Doppler (early, I know) so she sent me for an u/s which showed mm/c
2. 5w, RE. U/s in the ER for spotting at 11w. U/s at OB for 12w NT scan.
3. Civilian this time, but had a dating u/s at the OB at 6w.

Ah, thanks. I didn't recognize the term. Probably because we declined that kind of testing.
Standard for my practice is an 8-10 week nurse appointment with bloodwork and a 10-12 week OB appointment. The only standard u/s is the a/s. With this pregnancy I also had a dating u/s due to conceiving during a weird cycle and an NT scan. With DD I also had a viability check (they couldn't find the heartbeat on the doppler) and NT scan.
