Nope, never. But it's a lot easier not to with online banking and online bill pay. It's not like I write a lot of checks every month and have to keep track of whether or not they've cleared.

I voted it wasn't my fault, I swear.
A college job's payroll check bounced after I deposited and spent part of it. I got the overdraft notification and absolutely flipped out. I honestly thought I was going to jail and the bank would be calling the police any minute. I went straight to the bank a sobbing mess and asked to plead my case. The manager thought I was insane, took pity on me, gave me tissue and candy, and waived the fees.

Never....especially since we can't have overdraft protection on our account. But my FIL, on the other hand, apparently had $2,500 in NSF fees in one month. Uhmmmm....
I have overdraft protection on my bank account and if I don't have enough in checking to cover a check, my bank will cover it and then take the money from another of my accounts the next day. I do that probably once every two years, but I don't imagine that's what you're talking about.
I've never written a check and then ended up not having enough money in my accounts at that bank to cover it. I wasn't born MM, but my father did a good job of making me scared to death of extra fees in the form of overdraft fees, credit card late fees and/or interest,* and library/video store fines. I would be a lot madder at myself if I had a $20 overdraft fee than if I lost $40 on the street.
(*I do forget to pay my credit cards that can't be autopaid fairly often, but on enough of a rotation that I'm always able to talk myself out of fees. I need to cancel some cards.)
It was when M was a few months in dc, I couldn't remember when I was writing tuition checks and two hit at once. Our checks are split into different accounts so we had the money just not in checking. Other than that I think I over drafted one time in college.

Once and only once. I went to the on-campus ATM my freshman year of college and hit "200" instead of "20." I don't know why the machine let me overdraft but I went to the bank Monday morning with the cash.
I don't think I have ever over drafted even when my shitty employer didn't have enough funds to cover my paycheck pre-2004. All of us would head out to the bank ASAP to try and be the lucky few that had their checks clear. Good times.

It's been 10 years or so. The bank just took it out of my savings and charged me like $25 in fees. I almost had an issue a few years ago, but transferred money in time.
Pretty much the only time I ever use my bank account is to pay rent and credit card bills. It's not too complicated to check the bank account before that... So never.

I did once, in college. I swear my mom told me she deposited $200 into my account, but she only deposited $100. I bounced a check to a local store and I was so embarrassed. That was over 20 years ago.

I voted not in 5 years, but I also could go with "not my fault" The story:
When my exH and I split up in 2007, I opened an account with the same bank and transferred half our money out of our joint account (this was agreed upon.) What I didn't do was take my name off exH's account. So he was a pillar of financial responsibility and overdrew his account within 3 weeks. It stayed overdrawn, and finally BofA pulled about $2K from my personal bank account because my name was still on the account. So, I had to borrow money from my mom and promptly closed the accounts. He never paid me back, but that's not surprising.
DH has a thing set up with USAA if he overdraws, they charge his credit card that is tied to the account. Apparently he had this problem often before I met him.

The closest was when my accountant paid someone else's taxes (much more than mine) with my info. (Long backstory that makes this not as bad as it sounds lol).
I'm still not used to paying daycare by cheque and moved money to savings before seeing if Decembers cheque cleared. It hadn't. Thankfully we have overdraft protection so it drew that money back from savings, plus a $5 fee.
About a year ago I accidentally wrote a check from the wrong checkbook and overdrafted. It was such a stupid mistake and I actually caught it before the guy I wrote it to knew it would bounce but after he cashed it, so that was frustrating.
Before that, it had literally been probably 10+ years.
When I was in college someone used my debit card number to pay their Comcast bill. I didn't know the $200ish had been taken from my account until I logged into online banking and saw that I was in the red. So it totally WASN'T my fault and the bank refunded the overdraft fees.
Red Mill burgers are worth $500. Easily. I'd be in so much more trouble if they took cards, since I pass it at least twice a day. Good thing I don't carry cash or checks!
Like 5 or 6 years ago? H's dad was in and out of the hospital multiple times for months then into nursing homes then finally hospice. We visited him a lot and ate out all the time. Somewhere in the middle of that, we overdrafted the bank account. I admit it was my fault for not looking at it. There were 10 overdraft fees; I argued with the really awful bank manager a long time to finally get TWO fees removed.

I've only done it once and it was a few months ago. I set up my credit card to pay and it has always, for years, defaulted to the due date, apparently chase changed it to default to today's date and I missed it thankfully it just pulled from my backup savings but logging in to see "0" balance about gave me a heart attack
In college. My paycheck didn't get deposited when I thought it would and I didn't monitor my account and used my debit card. $25 fee for $2 purchase. Ouch!
I know I've done it but I seriously don't remember how long ago.
When we moved our money into the joint account, I used the wrong debit card by accident, and that was the account that didn't have money in it. I don't think that really counts.
I did sometime in college...I don't remember exactly when but I'd say it was at least 12 years ago.
I have had overdraft protection ever since. I do tend to keep a low balance in checking so I have accidentally dipped below $0 a few times due to forgetting something or miscalculation. It always just pulls from savings when I do that. I'd say I do that about once every 1-2 years. I can't recall if I did it at all in 2015.

I went with "not my fault." I have a credit union account that I use for fun money. My debit card # was stolen and they racked up a bunch of charges and overdrew the account. The CU waived all the fees.
Only once, I blame my mother and it was in HS. I wrote her a check and dated it for the next week after my paycheck would have cleared. It got mixed in with a deposit she sent to the bank with my dad the next day. She paid the fees.
